Supported Living and Outreach

We provide support to people who need extra help to live a more independent life by providing choices and opportunities to learn new skills and achieve their goals and dreams. We use positive and pro-active strategies which involve a collaborative team striving for the best outcomes for those we support.

Those we support can have a variety of conditions, with some having autism, a mental health issue, and/or those with learning disabilities. For these people, their lives may have been restricted in the past due to restrictive practices, overuse of medication and lack of access to specialist services. They may have experienced trauma and require time to recover and re-build trust and it’s vital that their experiences are considered.

Each individual has their own bespoke support package tailored to their needs, which includes person-centred Behaviour Support Plans and goals. We have been able to support those with more complex behaviours of concern to move from more intensive and restrictive settings into their own homes in the community. With high quality support and specialist input, we are able to successfully transition individuals to their homes and reduce placement breakdowns.

Independence and Quality of Life

Skill building is at the heart of what we do so that those in our care can become more independent and have a better quality of life. Having more independence will accord more dignity and allow the individual to expand their experiences.

Using person-centred planning and the PBS Framework, we work closely with the people in our care to get to know them. We spend time joining in their activities, hearing from their loved ones, learning about their likes and dislikes, offering choices and exploring new experiences. Where possible they are involved in making decisions about their support, with adaptive technology and visual supports to help communication as needed.

At Community Living Ltd. we do a thorough assessment of every individuals’ behavioural patterns, strengths and weakness to come up with a personalised list of attainable skills that would take them towards social independence over time.

We are passionate about seeing the people we care for achieve and grow – whether this be learning how to follow a new recipe, to cross the road safely, or have their first hair cut in the community. With the use of evidence-based assessments we can select those areas that need some extra help and engage in data-led decision making to monitor progress. Other areas of skill building could cover:

Some of the areas we are currently supporting our Clients include communicating wants and needs; showering with more independence; making meals of their choosing; learning how to respond to body language; having a conversation; making friends; budgeting; trying new hobbies and activities; and much more!

Behaviours of Concern

Some of the people we support present with behaviours that some describe as challenging to themselves or those around them. These behaviours can be distressing, can cause harm, and can limit access to events, the community and a good quality of life. We aim to spend time with those we support, to learn about what their behaviour means and how we might be able to help. Individual support plans, care plans and behaviour support plans are designed to ensure that the person is supported in the right way that works for them and those around them. By teaching new skills and training staff teams we often see a reduction in behaviours of concern without requiring direct intervention. Where direct intervention is required, we have a Behaviour Team who will conduct functional assessments (to understand the behaviour in more depth) and design specialist interventions whilst training and supporting the teams and families.
