Assessment Process
Our team can undertake a comprehensive assessment of an individuals support needs.
The aim of this assessment is to identify the level of support an individual would need to live independently in the community. Where an individual has identified Personal Care needs then this support will be provided by Community Living Ltd who are our CQC licensed operator.
A full outcome report will be written and presented to the participant and a copy of the report will be presented to the referring authority if that is appropriate. The Assessment process usually takes around 60 minutes and involves discussions with an individuals current care team and family members and time spent with the individual themselves. Further information is often requested so that we can ensure that our outcome is as comprehensive as possible.
